Position: Success Coach-Case Manager
Pay rate: $15-19.50 per hour + mileage
Part time: 10-20 hours per week
Job duties: Help clients with the following:
--Reviewing and assist in implementing parts of their case plan.
--Checking on your clients at least 1 or 2 times per week. (Depending on the
size your caseload)
--Attain state and federal benefits (SSI, Food, Cash assistance, etc)
--State and local benefits (State ID, Driver's License, Utilities,)
--State subsidies (Utilities-PIP, Cable discounts, Cell phone)
--Set up client appointments (Medical, dental, vision, therapy, etc)
--Driving them when necessary.
--Help them in getting bus passes.
Requirements:
--Compassionate. A desire to help others.
--Good attitude. If you have a bad attitude and are easily offended, please do NOT apply.
--Resourceful. You know how to find things. You don't give up with the first "no"
--Committed to your clients.
--Ability to multi-task.
--Ability to take notes and keep yourself organized
